One of the defining characteristics of DLB is the fluctuation in alertness and attention. An individual with DLB might experience episodes of being alert and engaged, followed by periods of confusion, lethargy, or even staring into space.

These fluctuations can be unpredictable, varying in frequency and duration. They can occur over the course of a day, or even within shorter time frames. This fluctuation, coupled with cognitive impairment, can be one of the most distressing aspects of DLB for the individual and their caregivers.

Fluctuating alertness and attention can also pose practical challenges, impacting the person’s ability to engage in conversations, follow instructions, or perform daily tasks. As such, these symptoms can significantly affect the person’s independence and quality of life. (6)
