This pain is a direct consequence of the irritation caused by the acidic environment in the stomach and intestines.

If ketone buildup in DKA is not promptly addressed, it can lead to severe complications. The most immediate threat is the worsening of acidosis, which can culminate in a life-threatening condition called ketoacidotic coma. This occurs when the brain, affected by the acidic environment and dehydration, ceases to function correctly. Other potential complications include electrolyte imbalances, particularly of potassium and sodium, which are vital for nerve and muscle function. High ketone levels can also exacerbate dehydration, as the body attempts to excrete excess ketones through increased urination, further worsening the overall condition.

Managing ketone buildup in DKA involves a comprehensive approach. The first and most crucial step is the administration of insulin, which helps to reduce blood glucose levels and slows down the production of ketones. Rehydration is equally important; intravenous fluids are usually administered to restore fluid balance and correct electrolyte imbalances. It’s also vital to monitor and treat any underlying causes that may have triggered the DKA episode, such as infection or inadequate insulin administration. Continuous monitoring of blood ketone levels, along with glucose and electrolyte levels, is essential to gauge the effectiveness of treatment and guide ongoing management decisions. (4)
